The Care Quality Commission (CQC) Registered Manager Fit Person Interview is a central stage of the healthcare and adult social care provider registration journey. Under Regulation 7 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, the registration inspector must be satisfied that the nominated manager is of good character, physically and mentally fit, possesses the necessary qualifications, skills, and experience, and is capable of managing the day-to-day regulated activity safely and effectively.
Drawing on practical experience supporting candidates through the registration process, we know inspectors do not look for memorized statutory definitions. They evaluate practical care management reasoning, leadership resolve, and your instinctive response to real-world care situations.
1. What Qualifications and Evidence Are Required?
Before the interview is scheduled, CQC assesses the candidate’s application and supporting documentation:
- Qualifications: Possession of or active enrollment in the Level 5 Diploma in Leadership for Health and Social Care and Children and Young People’s Services (Adults’ Management or Adult Social Care), or equivalent professional health and social care qualifications.
- DBS Countersignature: An Enhanced DBS check countersigned by the CQC or subscribed to the DBS Update Service within the statutory timeframe.
- Employment History & Gaps: An unbroken, accounted-for chronological employment history with full written explanations for any career breaks, in accordance with Schedule 3.
- Satisfactory References: Written references from past employers, including the most recent health or social care employer.
2. The Five Pillars of the Interview
Interviews generally last between 90 minutes and 2 hours, conducted via Microsoft Teams or at a regional CQC office. Inspectors formulate their inquiries around five core operational pillars:
Pillar 1: Understanding Regulated Activity & Single Assessment Framework
The inspector will test your knowledge of what constitutes Personal Care. You must clearly explain:
- What tasks constitute personal care (assistance with washing, bathing, dressing, continence, eating)
- Activities exempt from CQC registration (e.g., purely domestic cleaning, companionship without personal assistance)
- The 34 Quality Statements and how you will monitor compliance continuously.
Pillar 2: Safeguarding Adults at Risk (Regulation 13)
This is a core domain. You will receive an open-ended practical scenario, such as:
“A care worker contacts the office at 19:30 on a Friday evening stating an elderly client living with moderate dementia has unexplained bruising on their forearm and appears unusually withdrawn in the presence of their primary family carer. What immediate actions do you take as Registered Manager?”
Your answer must cover:
- Ensuring the immediate physical safety and medical assessment of the individual (calling 111/999 if trauma is suspected).
- Documenting objective body maps and factual statements without leading questions.
- Multi-agency notification: Submitting a formal Safeguarding Alert to the Local Authority Safeguarding Adults Team (SAB) within their prescribed threshold (typically same day / within 24 hours).
- Submitting a statutory Regulation 18 notification to the CQC via the online provider portal without delay.
- Internal risk assessment, review of care plan, and staff support.
Pillar 3: Safe Medicines Administration (Regulation 12)
Expect detailed inquiries regarding:
- The distinction between Prompting, Assisting, and Full Administration of medicines.
- Controlled Drugs (CDs) handling, storage, and witness recording in domiciliary environments.
- Managing Medication Administration Records (MAR charts) and eMAR audit trails.
- What protocols exist when a medication administration error or omitted dose occurs.
Pillar 4: Staff Recruitment, Training & Supervision (Regulation 18 & 19)
Inspectors will assess your commitment to safe recruitment:
- Strict adherence to Schedule 3 (two written references, verified identity, right to work in the UK, 10-year employment checks, enhanced DBS check prior to lone working).
- The Care Certificate framework and shadowing period (minimum 12-24 hours shadowed shifts before independent calls).
- Formal 1-to-1 supervisions (minimum every 8-12 weeks), annual appraisals, and observed spot-checks.
Pillar 5: Duty of Candour & Governance (Regulation 20 & 17)
How do you foster an open, honest, learning culture? You must demonstrate:
- Understanding the statutory Duty of Candour: Providing a formal written apology, full factual account, and appropriate support to the service user or relevant person whenever a notifiable safety incident occurs.
- Internal quality audit cycles: Monthly audits on medicines, accidents/incidents, care plans, call logs (monitoring late and clipped calls), and complaints log.
3. Top Mistakes That Cause Registered Manager Refusals
- Vagueness regarding Statutory Notifications: Not knowing when to submit Regulation 18 notices to CQC (e.g., serious injuries, abuse allegations, police involvement, death of a service user).
- Deflection to the Nominated Individual: Answering “the company owner will handle that”. As the Registered Manager, the legal accountability for day-to-day care compliance rests entirely on your shoulders.
- Reliance on Paper Templates without Policy Ownership: Failing to know what your own agency policies prescribe for whistleblowing or emergency call covers.
